Scheduling an appointment with our office will be simple for you. In order to arrange time for a visit or consultation, please contact us and speak to our friendly staff. We want your visits to our office to be as easy for you as possible and appointments are scheduled to meet your needs. If you are experiencing significant pain or have an emergency condition, every effort will be made to see you as soon as possible.

We understand that everyone has a full day and we want to make sure that scheduling time for you to visit our office is not a burden. The further in advance of your desired appointment time that you are able to contact us, the more likely we will be able to accommodate your wishes and schedule you at your most-preferred date and time.

We consider your first appointment with our practice an important educational visit for both the patient and the practice. We use this time to listen to your concerns and expectations. Upon reviewing your past medical history and determining your immediate treatment, further explanation regarding your anesthestic choices and possible surgical procedures will be provided. This visit results in a more predictable future surgical and anesthetic procedure and assists our staff in scheduling all patients for that particular day. If a surgical procedure can be undertaken during the initial visit, it will be done as long as it does not delay other scheduled patient appointments. More complex procedures will be explained regarding time and treatment needs and scheduled accordingly.

A parent or guardian is required during the initial consultation
if the patient is under the age of 18.

Please assist us by providing any of the following information for your consultation:

REFERRAL SLIP: Any and all referral request information from your referring doctor.

MEDICATIONS: Information pertaining to the medications you are currently taking.

MEDICAL CONDITIONS: Information pertaining to any medical conditions that you have had in the past or are currently experiencing.

INSURANCE: Please provide all medical and dental insurance information that applies to your account. This will allow us to assist you for possible reimbursement from the insurance companies that you are enrolled with.

X-RAYS: Please have them provided by your referring doctor or bring them in yourself. X-rays can be securely e-mailed from your dentists office to our office if needed.
